STUDENT ATHLETE MEDICAL FORMS
Below is a list of what is needed from student-athletes prior to their athletic participation at Skidmore College.  Necessary information is listed according to your class year, as the requirements are slightly different.  The Sports Medicine Department utilizes an online portal system for all form completion, which is also linked below.   Please check in with your Coach or a Certified Athletic Trainer (ATC) on campus for more specifics.

FIRST YEAR AND TRANSFER STUDENT-ATHLETES
First year and Transfer student-athletes all receive information from the Skidmore College Health Services department during the summer prior to their incoming year.  This information will contain all of the necessary medical forms to gain entry to school, which will include Immunizations, a Physical from their home physician, and a Sports Review Form, among many others.

 RETURNING UPPERCLASSMEN

• Health Review with Health Services on campus (schedule on your own time OR see Coach or ATC for scheduled times)
  • Sports Health History Form via Health Services portal MUST be completed before being eligible for a Health Review
• Forms via SportsWare online portal
• Baseline SWAY Test
